Does Cigna Cover GLP-1 for Weight Loss?

It will depend on the specific Cigna insurance plan. Cigna is an insurance company with hundreds of different insurance plans. Cigna is just one insurance company. As of 2026, the current state of affairs is that about 50% of Cigna’s employer clients offer weight loss GLP-1 coverage. The remaining half does not offer coverage for these medications at all or offers coverage for specific medical conditions, such as diabetes. Therefore, two co-workers at two different companies, both with Cigna insurance, could have very different outcomes with regard to coverage. 

However, Cigna has made significant strides in 2025 and 2026 to make these medications more accessible and affordable to those plans that do offer coverage. Cigna has partnered with Eli Lilly and Novo Nordisk to cap member out-of-pocket costs. Cigna also offers an in-depth lifestyle program, EncircleRx, with coverage for GLP-1 medications for weight management.

Which GLP-1 Medications Does Cigna Recognize?

Before going into the details of the coverage, it is helpful to grasp how Cigna differentiates the various GLP-1 drugs on the market, as the coverage depends on the drug prescribed, as well as the intended use.

This is an important point, which most people do not expect, but it is critical to understanding the coverage of these drugs. Cigna, as well as most other insurance companies, considers the drugs used to treat diabetes and weight loss to be two different categories, each with different rules, even if the drugs have the same active ingredient. While both Ozempic and Wegovy contain the active ingredient semaglutide, Cigna has a completely different policy for the treatment of diabetes, which covers Ozempic, compared to the completely different policy covering the weight loss version, Wegovy.

The insurance companies are very strict about the difference between the two, and an Ozempic claim will likely be denied if the diagnosis code is for obesity rather than diabetes. If weight loss is the reason you are taking the drug, then the appropriate weight loss product, which has been approved, must be prescribed, which is either Wegovy or Zepbound, but not the one approved to treat diabetes.

What Are Cigna’s Requirements for GLP-1 Weight Loss Coverage?

For plans that do include weight management GLP-1 coverage, Cigna’s prior authorization criteria – updated as of March 2026 – typically require all of the following:

BMI Requirement: You must have a BMI of 30 or higher to be classified as obese or have a BMI of 27 or higher with at least one documented weight-related health condition. These comorbidities include high blood pressure, type 2 diabetes, sleep apnea, cardiovascular disease, high cholesterol, and more. 

Documented Prior Weight Management Efforts: To get approval for your first prescription of GLP-1, Cigna will require documentation that you have been part of an intensive weight management program for at least three to six months. This is known as “step therapy.” This documentation must be part of the prior authorization request.

No Concurrent Use of Other Weight Loss Medications: New Cigna policies for 2026 have been issued with the inclusion of the following requirement: Cigna will no longer approve the use of GLP-1 medications when used in combination with other weight loss medications. Only one GLP-1 or GLP-1/GIP agonist will be covered at any given time.

Continued Therapy Requirements: If you are on a continuous course of a GLP-1 medication, then Cigna demands proof of clinical progress. As for liraglutide (Saxenda), the policy declares that if a patient does not experience a significant decrease in BMI after 12 weeks on a maintenance dose, then the medication is to be discontinued. The medication is then to be re-authorized on an annual basis.

EncircleRx Program Participation (for eligible plans): EncircleRx is an inclusive weight management program launched by Cigna Healthcare on January 1, 2025. It includes a mandatory lifestyle program for those patients taking GLP-1 medications. For those whose insurance includes this program, patients taking these medications for weight loss are required to take the Omada digital lifestyle program. The results showed that 92% of integrated patients remained on the medication after 12 weeks, compared to an average for GLP-1 medications. 

Cigna’s New $200 Out-of-Pocket Cap Deal

One of the most notable developments in the past year or so for Cigna members has been the announcement in May 2025 of a partnership between Cigna’s pharmacy benefits division, Evernorth, and both Eli Lilly (Zepbound) and Novo Nordisk (Wegovy).

Under the new partnership, employer-sponsored health plans can limit the out-of-pocket costs of their members on these two weight loss drugs to $200 a month, down from the $1,000 to $1,350 list price of the two drugs. In addition, members of employer-sponsored health plans who are currently taking the two drugs will be able to reduce their copays from as low as $25 a month.

The partnership aims to encourage employer clients who were on the fence about offering GLP-1 weight loss drugs due to the high costs of these drugs, but will now be able to make the coverage more affordable for the members they sponsor, due to the $200 out-of-pocket limit and the discounted price from the two manufacturers.

This is an important change, and this change is only relevant to you if the employer chooses to add this coverage. If you have an existing plan that does not have coverage for GLP-1 weight loss, the agreement does not change that. The employer must choose to add this benefit. 

What About Cigna Medicare Plans?

If you have Cigna Medicare Advantage, including Part D coverage, the coverage for GLP-1s is the same as all other Medicare Part D drug coverage.

Currently, Cigna Medicare Advantage does not have coverage for GLP-1s for weight loss because the federal government does not allow Medicare Part D to pay for medications to help with weight loss. However, Cigna Medicare Advantage coverage is available for the following indications: 

  • Ozempic and Mounjaro for type 2 diabetes management
  • Wegovy for cardiovascular risk reduction in adults with obesity or overweight and established heart disease
  • Zepbound for moderate to severe obstructive sleep apnea in adults with obesity

Medicare Part D recipients, including Medicare Advantage recipients with Cigna Medicare drug plans, will be able to get Wegovy and Zepbound for weight loss at a $50 copayment per month after July 2026 through the Medicare GLP-1 Bridge Program. In January 2027, more Medicare recipients may be able to get GLP-1 weight loss drugs through participating plans under the BALANCE Model.

Does Cigna Cover Ozempic for Weight Loss?

This is one of the most common questions, and the answer, in every instance, is a resounding “no” for weight loss. Ozempic is approved by the FDA for type 2 diabetes management, not weight loss. Cigna has a very specific formulary policy around Ozempic, and it does not qualify under weight management, even though it does qualify under diabetes GLP-1 management.

So, if you’re prescribed Ozempic for type 2 diabetes management and weight loss happens to be a side effect, then yes, Cigna will cover Ozempic under your diabetes management plan. However, if you’re not diagnosed with type 2 diabetes and your doctor decides to prescribe Ozempic for weight loss, Cigna will deny your claim.

The right way to use Ozempic for weight loss is under a different brand name, Wegovy, which has been approved for obesity management, i.e., weight loss, and this is the one covered under weight management, with all the prior authorizations we’ve previously discussed.

What to Do If Cigna Denies Your GLP-1 Claim

A denial isn’t the end of the road. Here’s a practical step-by-step approach if your GLP-1 claim is denied by Cigna:

Step 1 – Understand exactly why it was denied

However, you can request Cigna to provide the specific reason in writing for the denial of the claim. Some of the reasons include the fact that the drug is not on the plan’s formulary, the prior authorization was not submitted, the clinical requirements were not met, or the plan does not cover weight loss drugs. 

Step 2 – Work with your doctor on the prior authorization 

In case the reason for the denial is due to the lack of documentation, the doctor can submit the prior authorization request with better clinical documentation, including the patient’s BMI, the patient’s health conditions, and the patient’s past weight loss attempts.

Step 3 – File a formal appeal 

Every insured has the right to appeal the denial of the coverage. You can request that Cigna, in writing, provide the formal appeals process. Your doctor should provide a letter of medical necessity to support the appeal, stating the reason why the GLP-1 drugs are the next appropriate treatment in the patient’s medical condition. 

Step 4 – Ask HR or your benefits administrator 

If the employer does not offer any coverage for GLP-1 weight loss, escalating to your HR department may motivate a review of the benefits offered. Employers have the ability to offer GLP-1 as a benefit, and advocacy for employees may make a difference, especially with the new cost-capping options through Cigna and Evernorth.

Step 5 – Explore the manufacturer savings program 

Novo Nordisk (Wegovy) and Eli Lilly (Zepbound) offer patient savings programs for those without adequate insurance coverage. The programs are available for those with low incomes and inadequate insurance coverage.
